Metal Valve Stem Mounting

So I bought some of these outer mount metal valve stems before installing my new tires on the trailer. My local shop isn't super easy to get a trailer in and out of so I took off 2 wheels at a time to get new tires installed. After 2 different techs took care of me and I got the 2nd set home/installed I noticed that the rubber grommet had been installed on one set but not on the other.

My question for any tire pros out there...which way is right? The rubber grommet in the product pic seems to be for a special application and optional, correct?

I would say that the outer grommet should be used any time the mounting surface is not flat.

Side note - why in the world would you put steel valve stems on a boat trailer?

Actually the answer is here:

"For use with .453" and .625" rim holes."

You select which grommet to use based on the rim hole size
